Failings in Blackpool Council's Home Support leads to death of 79 year old lady.

Ombudsmans report highlights the council's failure to recognise risks that lead to the death of the vulnerable service user.

A Home Support worker did not visit the service user as they were required to by the council's care plan. The service user had a fall that went unnoticed until the police and ambulance service attended her home. The 79 year old lady was taken to hospital and sadly died 8 days later having suffered a stroke, a heart attack, and hypothermia in the hours after she had fallen.
